Thank you for choosing Green Park Lutheran School.

To begin the process, please schedule a tour.  A link to apply will be sent following your tour.  If you have already toured, please email Katie Gaeta at katie.gaeta@greenparklutheranschool.org to request the application link.

Upon acceptance you will receive an email inviting you to complete the enrollment and billing process.

Green Park is owned and operated by Christ Memorial, Gethsemane, Peace, and Resurrection churches. Families who are members of the association churches are supported by the churches. In order to receive the tuition subsidy please complete the endorsement form and give to your pastor or church office for process.

Church Endorsement

All new students, kindergarten, 4th and 7th grade students will need to have a full physical examination and current immunization record on file prior to the first day of school. We will accept the above form or your physician’s office report.

Physical Form

This form should be completed yearly by the student’s physician and signed by a parent/guardian for students to take any prescription and/or over the counter medications during the school day (one form per medication please.) Prescribed medication should be in the original container from the pharmacy, label attached, and currently prescribed for the student to whom it will be administered.  Any over the counter medications should come in the original container and marked with student’s name.  No medication (over the counter or prescribed) will be given at school without current written instructions.
